ADVANCED DIRECTIVE NOTIFICATION

You have the right to present an advance directive or receive information about advance directives. In the event of an emergency we will initiate life saving measures and transfer you to an acute care facility. Our contracted transfer facility will honor your advance directive.

If you wish to complete an Advance Directive, copies are available at the Center upon request.

We strive to reach your expectations at Timonium Surgery Center. If however, you feel that they have not been met, please call 410-560-3301 and ask to speak with our Administrator, Misty Smith.

If you wish to file a complaint, you can contact OHCQ, 7120 Samuel Morse Drive, 2nd Floor, Columbia, MD 21046, (phone: 410-402-8040 or visit OHCQ website.). Medicare beneficiaries can contact their Medicare Ombudsman by calling 1-800-633-4227 or visit their website.

After surgery, Timonium Surgery Center will submit your claim to your insurance company per information given to us. Any changes should be reported to our billing office as unpaid claims will be billed to the responsible party. Once your insurance processes, any patient responsibility due in excess of what was estimated at the time of service will be billed. Any monies collected on the day of your surgery are considered an estimate; the final calculation of patient responsibility is determined by your insurance carrier.

Please note that in addition to the claim for our facility, your surgeon will also bill your insurance for his/her services. In addition, any anesthesia services provided will also be billed as well. Some patients may receive durable medical equipment supplies (i.e., compression stockings, crutches, wraps, etc.) at the time of service; those vendors will also bill your insurance.
